I have only been able to accomplish this a few times. It takes quite a bit of self control:

You basically have to ejaculate without orgasm (99% of the time they are simulataneous, but they don't have to be).

I have only been able to get this to happen if I have been stroking for a while and have gotten a ton of precum flowing.
