It hurts far less than you'd think. There's a lot of bleeding because it goes through a lot of blood vessles but there's little actual pain. The nipples were significantly worse than even the apadravya. Fucking works the same way, there's just a thing sticking out the bottom of your cock. If the shop is doing it right they wont let you put jewelery that's so small it will tear in the piercing.

I have a double PA...one hole is a 00g and the other is just to the point where I can sport an 8g. I did them both myself and love'em!
