That's probably a larger than avg cock, but that's also a bit of forced perspective - the same thing they use in movies to make people seem like they are much larger, or smaller, than they are, like in "Lord of the Rings" movies.

Looks pretty real to me. JPEGSnoop didn't find any obvious issues with it. I think it's just the angle forward of the dick as well as the leaning in by the photographer. It's a big one regardless, but this particular situation exaggerates it for sure.
