When I experimented with pain, one of the first things I did when I was 15 was to pull sewing needles through my nipples. I did it a few times. Funny to see the needle point pressing from inside getting through the skin which really cracks as the needle gets through.

i was interested in piercings very early (especially ear an nipple piercings). the first thing i did was to push a needle trough my left nipple because i really wanted a nipple piercing, but my parents didnt want me to have it with 14... so i often pierced it just for me. i liked it very much to see the needle coming throug. when i was 18, i immediately got my nipple pierced properly, and i still like to wear it. the thing was not about the pain, but about the piercing, about seeing the needle coming through. but in reason, the pain came together with the wished effect, i didnt fear the pain. i can not say if i liked the pain, but it was no turn off for me.

i never tried any needle play, but i think, i would not be averse from that. i still just have the left nipple pierced, so maybe i will try it with the right nipple one day.

Hmm, I don't think so. Needles are the only way I can think of to make it permanent or precise. At least until there is a laser or some such thing. But that's probably years down the line.
